Records case histories over time, including patient information, risk management reports, medical records problems, patient mortality, medication use, utilization review and top three DRGs.

Physicians are rated as follows:

• A = Management was appropriate
  
  
• B = Standard of care was not met
  
  
• C = Substandard care with adverse outcome
  
  
• D = Standard of documentation was not met
  
  
• P = Pending
